While it would seem obvious that Ros is the killer, it would make more sense from circumstantial evidence it was her husband or someone else.
1) The murders started before she took the job.
2) The killer was supposed to be strong man.
3) Coll had the hammer in the end when he rescued her.
4) Who killed the older nurse in the stairwell?

I realize all that but I am pointing out how that was not obvious because it didn't match the narrative from the rest of the story. Also, if she is killing other women, blond and ex patients, how did she know they were ex-patients if she was killing them before she even worked there?
