Probably the same reason not everyone is affected by the flu, things like this are random and won't affect everyone.

The film aims to the analogy of blindness as the passive-agressive living that torments this society; ambition, power, greed, lack of consciousness about the others and so on, the absence of productive leadership, complaints over options, justification of the poor moral quality. Even so, there are people, almost none, only one in this city, to our knowledge, who needlessly care about how things should be done, and get it done. That's the Doctor's wife's part.

Could you explain what you mean by "needlessly" care, please?

Not. Important.

A typical Hollywood film would have had scientists in a tent, giving stats about the percentage that aren't affected, wanting to get a sample of their blood to determine what made them immune (not to mention all the science around what the heck caused it in the first place), lots of politics, more scary military scenarios, etc. etc.

This wasn't that kind of film.
