I think it's a kind of surreal ending which is also played in the comatose guy's mind, as do all the scenes with the homeless guy. In his subconscious mind he is dreaming of desire, lust, company and connection to other people who care about him (especially the Bangladeshi worker and the girl who works in the cafe and takes care of him during his coma). Hence, the title of the film.
The ending was beautiful to watch and not a second too long. The brilliance of Tsai's work.
