Why does she add a picture to the photo album?

this one threw me off

It is her way of letting Julia know that she has a new "daughter". Also connect this to the "gain a son/daughter" speech near the beginning of the film.

I agree, jacowium. I also thought it was Louise's way of saying, I'm now a part of your life. It could have also meant that Louise was starting to share a true part of her story after Julia had shared with her. And I think the photo resembled some permanence in their being a part of each other's lives from now on. What a beautiful movie. :-)

Whilst I don't disagree with the opinions offered so far, I thought it was more Louise's way of saying:

- No, this photo album shouldn't end, sadly, abruptly. It should continue with a happy photo of happy memories to look back on - and not a blank page staring back like a life-long continuous regret. It also signified that Louise had moved on from looking out for No:1 maybe even looking out for Julia from now on.

Must admit, at first I thought it was Louise's own personal photo that she'd carried around with her, like forever, meaning that she was the daughter - but Louise didn't carry a handbag, only a backpack, and the purse came out of Julia's handbag.

I was also wrong footed in the final scene where I was sure Julia would be rewarded with seeing the tattoo she'd so longed to see as final proof.

A quite beautiful, poignant film, totally non-Hollywood, devoid of the high concept/low concept crap.

Don't you just love Sigourney Weaver in this?
