Ok so ocelot forms the original Patriots with Big Boss and an Unidentified individual who he talked to on the phone. Well I figured out who it was! It was Major Zero! Think about it, Gene calls the unidentified guy, "the man with the same code name as Null". Well in mathematics Null is commonly used to refer to Zero. Think about it

it could have been campbell. think about it, zero isnt mentioned at all in any games other than snake eater.
but campbell DOES taker over as the leader of foxhound, and continues with the story
hell even his likeness is on a computer program set up by the patriots.
so it could make sense that it was him all along.
but hey i didnt get any sleep last nite and it could just be that.

After the scene with the "same codename as null" i thought about it and came up with Major Zero in about a minute. First of all it's totally obvious, Null = Zero. 2nd there's like 7 characters that survived MGS3 anyway. Ocelot, Eva, Snake, Sokolov (although i though he was killed), paramedic, sigint, prolly someone i forgot too.
So obviously it's not paramedic, sigint, ocelot or snake (I think snake would know if it was snake). So who else is left? Eva =/= null (and that would be lame). Doubtful Sokolov would orchestrate his capture, torture etc, plus he's a lame ass. That leaves 1 guy - Major Zero, who is conviently absent (in prison) during the game.
Although if it turns out to be Major Zero I really hope they give him a backstory to figure out WTF was going on.
One thing you didn't mention is Ocelot talks about making "her" dream a reality. So I wouldn't be surprised if The Boss and Major Zero orchestrated this whole thing so that even though the boss would die, Major Zero could get the entire legacy, and then have Snake (or i guess Gene or someone else) actually fulfill her dreams. Wondering what ppl think about that?
I think you might be onto something with that, they were in battle together in that WWII area if I remember right, he could've been behind the plan all along
