Yes, it is all in his head probably form the point they enter the asylum. Notice that hte killings happen in broad daylight, and think aobut how much more sense it makes to take a suspected killer for an overnight trip anywhere.
The guy is obviously crazy, from the beginning. He killed his first girlfriend and the second. Also notice how clean the asylum at the end. This makes way more sense than a crime scene looking that horrible. And if there was a chair, it surely would have been removed as evidence. Also everyone was an extreme characterization, the annoying prick, the quiet drs assistant.
