Who is the lady at her house when the old woman comes back from doctor?

I am just curious about that.

One more thing, it seems that that lady talks about the piano before she is leaving and says if she can find a buyer, she will take it. However, the old woman says it is hers. I quite don't understand it. The piano is lady's or old woman's?
