How about in Dead Rising when a female zombie gets ahold of you and start trying to ummm I guess attack your crotch.
Or in Legend of Zelda on the N64 when you get attacked by the weird zombie like things and they jump on you and starting doing things that look.... Well just really weird.

Guess someone never played the Silent HIll series. For example, in Silent Hill (won't tell you which one) A man set himself on fire, screaming bloody murder and carved numbers on his chest while burning to death. Yeah. And someone gets killed by... a rabbit.
Most disturbing death ever!! Shadowman on the hotel level Mike opens a door out pops a bound and mutilated man still alive in agony either he dies after walking down the corridor or you can mercy kill him. Doesn't sound too bad until you remember that all around the building there is tape recordings of people dying and mutilated bodies everywhere *beep* me right up and stuck in my mind for years disturbing is not the word he he he
how about any of the execution kills in The Darkness, or the creeping dark biting peoples faces of in The Darkness, or the demon arm impaling people in The Darkness; and after you kill someone in The Darkness, you eat their heart to get stronger
Probably mario getting hit by a large Bullet Bill in the face, turn around and be blushing, falling down, pausing in mid-air going up scream in pain and fall again.
the part in "The Darkness" when the darkness is preventing you from saving Jenny and you see Uncle Paulie shot her in the head right after she says it's not your fault.

never has a game elicited so much emotion from me. i was shocked and sad alittle because I felt so bad for Jackie.
In the expansion to FEAR the leader of the Special Forces is mutilated fairly well. He's smashed up and down in a warehouse and thrown into the next room where you just hear screams and tearing and blood. Then when you find him he's pulled back and torn just a little more.
It was kinda annoying actually, since everyone who survived the actual game died in the expansion. Oh well, fun game.
put a freakin spoiler alert in yer post, COGSOLDIER. I beat the game, but it would suck having waited for 3 years only to have the ending ruined...

I always thought the sound made in Manhunt when Cash was stabbing a guy in the head with a crowbar was pretty gross... Sounded like he was pulling the bar outta a watermelon... all squeaky and wet sounding.
Also the chainsaw in GoW was rough I thought.
as a after thought... all deaths in Manhunt were grusome; hatchet, garret, meat cleaver,glass shard where you stab their eyes out... after the first eye, they kinda go limp in your arms and twitch a little), and the chainsaw in which you drive the reeving chainsaw through the top of their head into their chest. I think deaths like that are brilliant. I mean, someone had to think of it, right? [evil]

its not really disturbing but when Sgt. Johnson dies in Halo 3, and his last words were "Let me go out with a bang.", i almost broke down in tears.
Yeah i totally love that, so emotional and dramatic. I love it how when you take the spartan lazer from him while he's dying, he just whispers, "Kick his ass!" Totally awesome!
Phantasmagoria had the worst death by far!! The part in the flashback where the guy force feeds his wife their liquidised baby and she chokes to death on it.

Dead space: guy smashing his head off the wall repeatedly til he dies, or the nurse cutting into the guy on the table with a bone saw while he sreams then her sliting her own throat. or getting attacked by the dead babies who jump on your face and stab you in the back with the tentacle things, or getting hit by a meteor and being blown apart or generally pretty much everyway to die in that game.
Agree 100% about Prey, while not exactly the most gorey death, it's definately the most disturbing scene by far after playing games for 20 years or so lol
Never been actually bummed out by a game before like that, on top of that actually having to finish her off yourself. The sound helped alot too, entering the room and hearing the theme music, the pod opening and her/you screaming, then the conversation at the end, wow Definately not saying they shouldn't have done it, can't think of a single thing that would be better, but definately a downer. The very end made it a little less sad though
They picked a good one to voice the main character too, he's got really convincing horrified voice, given that you don't actually see his reaction it did the job well lol

I think GOW has some... I'm gonna say gory deaths because it doesn't disturb me, or I would of turned the blood off.
When you chainsaw some - blood blood blood! When you shoot someone's head off, especially with the sniper rifle. Blood sprays everywhere. When you blow someone's limbs off with a torque bow. That's just awesome! ..Or when someone falls to bits because you put serious ammo in them. Getting shreaded by the Kryll (is that how you spell it?) That remind me of Pitch Black.
Let's face it, the whole game is gory as hell! I've had it since last December and I haven't played online vs yet. Gonna have a go now! I've always kept off it because I know how good some people are at sniping, and getting sniped every minute annoys me lol. It must be satasfying killing someone on GOW
For me there's gory, and then there's disturbing. Sometimes they're the same, sometimes not. My list is:

DREAMWEB
The old PC adventure game Dreamweb, even though it's top-down and "old school" graphically, is very bloodthirsty (actually got "withdrawn from sale" here in Australia) and you play a guy who has to hunt down and kill certain people.
One scene has a woman blown in half by her security system (which you disrupt) and begging to be killed, another scene involves you hacking two bodyguards with a fire axe, then bursting in on a young rock star having sex in his penthouse and blowing his brains out!
But the reason this game has the "most disturbing death" scene for me is that while almost all the people you are assassinating are nasty pieces of work in general, the solution to one puzzle is to shoot a kindly elderly security guard in the face point-blank. There is NO other way past him, no other way to solve the puzzle. The old guy just has a chat with you, apologizes he can't let you past into the tv studio, so you just pull out your gun, shove the muzzle against his glass booth and blast him.

HITMAN: BLOOD MONEY
Among the last kills in the game are an innocent priest and a likeable reporter, both of whom are purely in the wrong place at the wrong time. Up until then, the Hitman games have gone out of their way to encourage the player NOT to harm innocents, and every target has been in some way "deserving". Not this time.
The game will NOT end until you kill these two, even after all the "enemies" in the level are dead. It FORCES you to kill them (most likely as they cower/try to flee), which is disturbing enough on it's own. Much like why I put Dreamweb above, it's the fact that the player has no choice but to kill innocent people - when the rest of the game is NOT about this - that makes the moment so disturbing and uncomfortable.

FINAL FANTASY VII
Sometimes it's not about blood and gore, it's about unexpectedness and fondness for a character.
I don't think I need to spoil this one to let anyone know who or what death I'm talking about, suffice to say I STILL hear people asking if there's a trick to save them, or anything they can do, or how to bring them back, etc.
Years of denial ("there HAS to be a way to save her") is a pretty good sign a death affected people dramatically!

The first big death in the game was not only arguably the most brutal and disturbing, but TOTALLY unexpected.
Everything from the game's manual and advertising to the opening scenes and plot set-up made me completely unready for that... you've been convinced that this character will play a big part in the game, then they are horribly killed right in front of you within minutes of the game starting proper.
From then on you realize that no-one is safe.

This one's all about the gore.
Sean the cult leader's death is one of the few video game deaths to actually make me go "Oh geez, that's NASTY!". Adam's was gory, yes, but so OTT that it was kind of funny (and a bit of a Batman homage, too)... Sean's was just OUCH!

The fact that you cannot save Angel, that you only find out what happened to her late in the game, and that the news comes right after a HUGE betrayal by another character all added up to one of the few genuine "KHAAAAAAAAN!" moments I've had in a video game.
Why couldn't the movie have been as good as the games? :)

I hardly remember this game as such, but I DO remember two things: that the training mission involves you killing (for real!) your fellow agents who aren't as good as you, and the first mission begins with a very disturbing moment of watching a fellow agent begging for his life before being executed mid-sentence.

Luca Blight forces a helpless captive woman - who begs to be spared - to crawl around on the ground squealing like a pig, making her think that this will save her... then laughs and says "Die, pig!" and hacks her to death with his sword!
Proves you don't need bloody gibs and 3D graphics to make you squirm at a death scene. It's often not what you even show, but the context behind it.

The deaths in the "torture brothel" place going on around you actually kind of put me off the game, meaning I never bothered finishing it. Only time I've ever felt "maybe this is going a bit TOO far", given how sadistic it was. Mind you, getting past the guard with the severed head made me laugh out loud.

SHELLSHOCK NAM '67
In particular, the deaths of the women near the end was very disturbing and brutal, IMO, but so were most other people's deaths in this game (eg. Psycho's death!).
I really enjoyed this game, but it really pushed the "war is hell" motif with some of the death scenes! :)
well definitly the saddest death I have ever scene is Eli Vance in HL2 Ep2. I've watched that scene three times now and still get serious goosebumps from it.
In Gears of War. They didn't show this but millions or billions of people being burned by the hammer of dawns is pretty graphic. Whoever survived the onslaught is now called a stranded.

I don't know if you would call this distrubing but Venom is defeated by being impaled from falling of a building in the Spiderman 3 game.
The most disturbing death in a video game for me was Emma Emmerich from Metal Gear Solid 2. After what seemed like one of the most difficult levels in any game I've played that SOB Vamp shows up and no matter how good of a shot you are he stabs her and causes her to bleed out.
Then the little scene with Otacon afterwards always pulls at the heart strings, I have a little sis too. When Metal Gear Solid 4 comes out I'm going to exterminate Vamp with extreme prejudice.
