I read in Full synopsis:

On a deeper level, it is an examination of US foreign policy, government error (or possibly conspiracy), and the demise of western civilization embodied in George Bush, whom the writer deems 'grievously' ill.

I thought it was a load of anti American, Anti Western liberal guilt bs

Not at all. I saw it as more of a take on the never ending debate on the comatose mind and when or if it is best to discontinue life support. We don't know if they are aware and understand the things going on around them. Are they in a peaceful state or trapped in a nightmare of their own making that can only end with death. I think it is easier to relate if you have witnessed a loved one in that state and had to make that choice.

I think the brother was kind of a foil for Tom Cavanaugh. He was this laid-back young guy traveling the world and doing good while George was middle-aged and living a totally boring suburban life, which ultimately fed into his insecurities about his wife.
