Does the book shed any light on why the brother died? He just up and dies, just like that? Why would they be transporting him if he was that sick?

Hard to say, but I have several thoughts that come to mind. Head trauma, an embolism, pulmonary edema. I guess we'll never know for sure.

He was being transported because his mother was under arrest. They weren't waiting around for him to be better before shipping her off to wherever they put Communists where she presumably died. She was going, so the children had to go to their foster home.
