I was just wondering if anyone knew if there was some kind of significance to him imagining spiders coming after him or was it just because he was afraid of them? Maybe that is mentioned in the book but I don't recall them saying anything about it in the movie. I hate them so I spent much of the movie hiding my face!

I haven't seen the DVD, but when he first arrives, the nurse wheels him into a room that has webs in the doorway, and then you see him trying to wipe the web off his face and a spider is in the web that he is trying remove.
