this is a super late reply. but I don't get this post... what does the character being nice and attractive have to do with her being in the "loony place"? is she supposed to be ass-ugly? is it not realistic for "attractive" girls to be in hospitals?

There's no reason for there not to be attractive girls in that hospital setting, but it's entirely predictable for a casting director to choose a good-looking (even if you don't find Emma Roberts attractive, the only people who would deem her hideous must have extremely high standards) person in the role that Emma Roberts played.

There might be some defence for this particular film as it is allegedly based on a book, so Emma Roberts may fit the description of the character. However if the criteria for the boy's love interest was merely someone his age, then a casting director choosing a thin, blonde, good-looking actress just reinforces the suggestion that movie companies has a specific perception of beauty and will cast accordingly.

This is a super late reply to your super late reply but at the time of my posting, the film "The Duff" has a classic example of this.

There are attractive and unattractive people in rehab and mental institutions. But actors are generally attractive.

you'd be surprised then that a lot of people in psychiatric wards are attractive.

At a guess you are probably butt ugly and think life would be easy if only you looked nice.
