Why is the female psychiatrist played by a man?
Am I missing some point they were trying to make?

i think the psychiatrist is a women, but tracy saw him as a trany or as a male, why? i don't really know why? i just know that the movie is made in the prespective of tracy.

tracey most definitely saw the psychiatrist as a female. Her voice over always uses the pronoun "she" when talking about the tranny. Not to mention the numerous female derogatory names tracey calls her.
Look at how tracey's acceptance of the psychiatrist changes throughout the film. In the beginning, she rejects her, both because her parents put her up to therapy and because she doesnt understand the tranny's methods. Tracey is a lot like the therapist: isolated (like their almost soothing blank white setting chose for the therapy sessions), unable to get through, and (ahem) missing some parts (both physically and emotionally in tracey's case). Tracey obviously wouldn't agree with this persona. She's horribly ashamed of these traits in the beginning. Towards the end, however, she wants to join the tranny, in hopes that it will fix her family and end her isolation. The tranny however, is now rejecting this idea, as that's just not how it works. And so goes the name calling as tracey has a sulking moment, only to get over it once she realizes it is her isolation and missing parts that make her unstoppable.

at least, thats my interpretation.

The therapist's appearance might be a projection of Tracey's "missing tits". If so it's a hint that her self image is very screwed up by the other kids constantly giving her *beep*

Can we please stop the use of the phrase 'tranny', it is offensive, and doesn't actual specify anything. Do you mean transvestite, transexual, transgendered - they are all different things.

I think that the mixed gender of the Psychiatrist, is a metaphor for Traceys own confused feeling about her gender i.e female feelings, but being mocked as a female and being treated as a male. The other students even use a non gender specific name for her "It's".

When she asks the Psychiatrist if she can stay with her, and don't send her to another psychiatrist she is saying that she doesn't feel ready to move on, and can't she just stay like she is/where she is developmentally. She doesn't feel ready to move on to womanhood which is why the curses that she uses against the psychiatrist are mostly female orientated.

I'm honestly not sure. Strangely enough, the actor who played the psychiatrist is not transgendered at all, he's married and has two kids, and usually plays male roles in Canadian film and television. Perhaps the book would elude further as to why the psychiatrist is transgendered. Tracey sees and refers to the psychiatrist as a female, which may either imply that it's a man working a "woman's job", or that it's a woman working in a career normally dominated by males. There is just so much nonsensical symbolism and metaphors in this film that it's difficult to say for certain. 😕
