How did Eva die?

My guess is the standoff stalled and Randall threatened to start shooting hostages one at a time if his demands weren't met. He probably reached a point where he felt that the negotiators didn't believe he would do it and he felt he had reached the end of his rope and decided to shoot one. I imagine that would Randall took his time to decide and emotionally tortured the hostages while making his choice. I also imagine that he argued with Lucas at this time as well and that his anger led with Lucas enraged him to follow through with killing Eva.

I just re-watched Act I of the Pilot and Jeremy clearly tells a member of the SWAT team that he has been treating Eva for the last hour and a half. So she was shot 90 minutes prior to the SWAT siege on the bank. Eva's shooting most likely led to the siege.

In What About Eva, Kathryn made a comment that gave me the impression that Eva snapped and became hysterical, made a run for it, or something. I need to rewatch, but she seemed to suggest that towards the end, Eva lost it. From that, I kind of assumed that maybe the tension, stress, fear, heat, etc. got to her and she sort of lost control. Randall was threatened and/or annoyed by this and with the rising tension and deteriorating conditions inside, snapped himself and shot her.

I wonder why it took SWAT so long to attack, though. Usually, when a gun shot is heard inside, SWAT goes in within a matter of minutes. (Remember how rushed and panicked everyone was after Tom got shot?) Why did they wait an hour and a half to go in at the end? More stall tactics?

It's just speculation though. We'll probably find out when the TV Guide guy does his Q&A.
