It really doesn't make sense that the man in the straightjacket would be a former patient. They said that when the hospital closed, many of the patients were released on the streets. I understand that some returned to the hospital and squatted for lack of anywhere else to go, but there is no reason why any of them would still be wearing a straightjacket. Even if the patient was dangerous, if he was being released, the staff would have removed the jacket. Even if they hadn't or the patient had escaped, that would mean that he had been wearing the jacket ever since. How did he eat or go to the bathroom and thus survive?

I think the "former patient" was only added for creepy effect, but had no real logical merit. It would have been cool had that been Jack in the jacket and he was seeing himself as he had been when he was a patient there. But the caseworker at the end found the guy in the straightjacket before she discovered Jack, so he could not have been in Jack's imagination as Charles and Emma had.

Is the man in the straightjacket literally there, or is he an externalization of Jack's memories & terrors? That's the question.

I have no clue what the movie was about. From the Plot it seemed interesting, but the direction was very poor. I have watched tons of movies on this subject, but this one really made me go to sleep. From the ending, it seemed the movie was about the infamous hospital where tons of abuses went on. I've seen a Ghost Adventures episode on it, which was very creepy. Apparently the hospital has now been turned into a Halloween Attraction.
