Cake's past

In the pilot/first episode, Joey and Looch tell Fran that since Cake refused to tell them anything of his past, or even his name, they called him Mangia Cake.

Please take into account that I missed the second episode.


So why did Stuart never tell them? Where did she come from anyways? Did she come to Toronto with Cake? Did they live in Toronto before? Why didn't O'Malley or Malta or Esposito tell anyone?

Or did I find a rosebud? >_>
