What did it mean when...

There are 2 things that I don't know, both happen when Josef is under Hypnosis, when he goes into a mental institue looking for Bertha a nurse tells him that he has to wait her upstairs, right after that there is a painting on the wall and he stares at it for a while, anyone knows tha painting and its meaning? And second when he goes upstairs the stairs leads no where and then he realizes of the door at his back, what did it mean?
