yea probably with fascination with death bc he sits and watches them die and when the docer and kate go upstairs to look at the old lady who had died he says well a lot of people have a fascination with death.

I think it was a combination of a fascination with death as others mentioned, and he seemed to enjoy the power he had over their lives and deaths.
And also he makes mention that he's 'freeing them from this life' which he seems to see as awful and miserable.

I think it was something like "Arsenic and Old lace," the idea that the old people were better off dead. The doctor even made a comment when he visited an elderly patient who had died, "so peaceful" he said to himself. He was fascinated with death and the old people fit the bill to fulfill that fascination and control. Another doctor playing God if you will. He was a definite sociopath.
