When the other lady at the hospital tells Karen about the suicide of Viviane, it looks like Karen is very upset, yet the lady says "You're one tough cookie." or something to that effect. What does she mean? How is Karen displaying 'toughness'?
"I'll book you. I'll book you on something. I'll find something in the book to book you on."
I can't remember exactly... but I think the other lady says, "you're not gonna take the subway tonight, are you?" and Karen says she plans to. I think that's why she calls her tough, if I'm remembering correctly.

My interpretation is that the other woman was encouraging Karen. Essentially saying "You're tough, you'll get through it" because it seemed like Karen had an attachment to Viviane whether it was friendship or a feeling of responsibility for her.
So saying "You're tough" isn't an observation about her current status, and how she's handling the news of Viviane's death, it's encouragement that she knows Karen will be able to get through it because the other woman knows that Karen is tough from past experience.
