throwing up in the street!! on duty!!

i have suffered from bulimia for many years and have just come across this show on youtube.
What i dont understand is why Adam throws up in the street, in broad daylight, whilst in his police uniform!
i have always managed to find a toilet whether it be a public one or my own. it makes the show a bit unbelievable. how would he clean himself up? why wouldnt he just use a toilet?
the shots of him alone in his apartment eating 2 meals is far more believable and conjures up the image of absolute loneliness and despair that a bulimic feels. it also shows the strange rituals that they abide by.
the street shots are just too unraelistic for me...
