What happened to Jack's mom?

We found out about Jack's dad....but I dont think I ever heard anything mentioned about his mom. Did she leave? Or die?

I might be wrong but my theory is maybe the whole "bad things happening to good people" comes from the death of Jack's mother?

Anyone have a clue what really happened to her?
