I believe the being in the winter coat was just a manifestation of the curse from all the disturbed souls trapped at the convenience store. If you remember, when the Eskimo, as you call him, is the customer his total comes up to 44,444 Yen (give or take a 4). This is part of the movie's play on cursing the people who get the specific repeated numbers (666, 999). The number "4" in Japanese is an unlucky number as the pronunciation of the word also means "death". So, by the Eskimo's purchase equaling such an exorbitant amount and it being all 4s, we can assume that he is Death; he's just the Grim Reaper in a more casual style. ;)

Nao is the main character, right? She's not the one who died in the beginning, it was a different girl. That girl appeared in the beginning just to give a shock to start off the movie. We find out later that the moment she died was after the events we've seen in the film but in the second time, you finally see what it was about the place that gave her the creeps. They saved that information for last instead of confusing the audience with it from the get go.

Why did the man in the wheel chair die? I don't know. It seemed to me he was depressed at losing his feet, so I assumed he committed suicide. He was broken up over the fact that everyone else had feet. As he said, ghosts don't have feet, so I felt in his sadness he took his life to go over to a world where he would fit in with everyone else.

As for the elderly lady, I'm pretty sure she was crazy BUT she also knew what she was talking about with the curse.
