It seems the winner is always an Iron Chef.
I do remember in the original japanese series, they would switch up winners, meaning sometimes the challenger would beat the Iron Chef.

There's been a few who succeeded- Anita Lo, April Bloomberg (one of the very few who managed to beat Michael Symon), Traci Des Jardins (Mario messed up badly with his final Shrimp course where all the judges HATED that dish) and I'm sure there's others. Those 3 women come to mind especially
