What was the deal with Gregory's lip wound?

He received it being beaten by fellow warders but it never really healed over the following two decades. A scar could be expected but it looked bloody even later when he had grown a moustache.

I thought that maybe when he got hit the punch actually connected and he was left with the cut as it appeared to be healing up by the end of the movie.

Maybe the idea was that it somehow developed an infection or something...
