When maniwa became all delusional in episode 7 and was fighting littel slugger in the beginning of episode 12 did littel slugger really give maniwa that wound with his bat or did maniwa hurt himself on purpose to make his delusions real?

shareIt seems to me the "delusions" ended becoming so strong that they ended up being real. That's why in the last episode they talk about the recontruction effort on the radio that was the result of the damage from the black blob. So I'm pretty sure the wound maniwa got was real and it was from the little slugger entity.

Lil Slugger and the wound he made were real. So I guess that means the doll's were real as well, if they were actually able to treat the wound the way they did.
