Lots of spoilers follow:
I just finished watching this 15 minutes ago and came on here to find out what happened at the end because I wasn’t sure I understood it and of course no one else knows either... but anyway, I think they tried to imply that Tom may have bludgeoned Barry in the head with his guitar as the two jumped into the screaming audience after one of their songs. This knocked Barry out and after he woke up he never spoke again. Tom basically snapped, due to pent up rage (directed at Barry) and possibly sustained drug use, saw his opportunity to take Barry out or merely did it spontaneously. So the boys are brought back to the Head by their sister where they live for who knows how long. Tom mumbles, Barry says nothing. The sister finds their bodies outside one day, their arms wrapped around each other. She said it wasn't clear whether Barry had died first, but she could see where Tom had tried to cut himself free of Barry. So maybe Barry died a short time after he got hit in the head from the injuries or maybe he died later from his heart condition or maybe they both died while trying to cut themselves apart. They do hint earlier in the film that maybe Tom would have been more keen to separate himself because he says in an interview that he would separate himself if Barry were dying and Barry says it would be better if they died together. They don't explain why the two of them would have been outside when they died. But, they do keep replaying this audio recording of Tom, perhaps under hypnosis or while sleeping, where he talks about seeing sand and asks where Barry is. I guess this could be seen as a premonition of sorts to their final fate. Barry dies and Tom is left alone. I don't know. If you watch the film and have any ideas I'd like to hear them.
