I have listened to this exchange over and over and still cannot understand one word of that particular phrase. it's driving me nuts- hope someone can help out- !
I totally understand your confusion - halfway through the film I put subtitles on!!! What she says is "Do they make you do aerobics in the pool? What happens if you don't want to"? (he had just said that could be an option on her vacation)

it turns out not to have been Of Huge Significance, this time.. other than to show Hannah couldn't be less interested in a conventional 'leisure activity filled' holiday (to put it mildly!)

in my opinion this line was of significance. I interpreted it as "there's something wrong with my body so I can't wear a bathing suit". Later we know why and at that earlier point the line arouses curiosity like so many other "neurotic" details about Hannahs life.
I've always taken the line to show how little the boss understands in this conversation. He's trying to tell her what a wonderful time she'll have, but she only sees the "vacation" as things she's going to be made to do against her will. (That's why the boss sighs in frustration in the next shot.)
