Maybe this is completely obvious, but after Walter falls over and we see the cast and crew watching the movie, are we in a completely different story? Because the director talks about the scene with 'Steve' and 'the baby who plays Tristram'. Plus other things, like how 'Steve's girlfriend' was in the movie. I don't really see why, if this is indeed a third layer, adding another level was helpful/necessary. Anyway, any thoughts?

You are absolutely right! I didn't understand that why Steve tells her girlfriend that she looked great, but thanks to you I got the ending now.
And I think it's actually a good twist, I mean without the ending it was supposed to show the making of an actual Tristam Shandy movie, but the ending explains that they didn't want to make a movie about TS, but they wanted to make a movie about making a movie about TS. :)
Yes, there is another layer. The girl playing Steve Coogans girlfriend is actually saying: "I forgot how short I am." and Steve Coogan and her are kissing akwardly on the cheeks after the screening.
