Hmm... I'm not sure what "twist" from this movie you're referrin to but I shall pick out some key points anyway ...

*SPoilerz here*

The 'mechanical girl' aka. the creature of the show is no girl but a woman named Charlotte ... she was once a nurse of the same hospital the lead character, Amy (Calista) works in ... she was too attached to the children, one girl Mandy in particular who had the brittle bones disease ... when Mandy starts to get better, she is afraid that the child will leave her one day so she suffocates her and commit suicide (eventually after the hospital board tells her that her services had been terminated)... she was the 'mechanical girl' because she put the kind of metallic devices worn by Mandy for walking support before jumping out of the window... she continues to roam the hospital as a ghost/creature in those metallic stuff (beri freaky and grosteque)... the unexplored floor of the hospital is the level which Mandy use to stay at and receive treatment... they shut it down after this tragedy but Charlotte's ghost stays on ...


------ in the end, the creature/ghost of Chorlette was satisfy only after claiming one of the kids, Maggie (who has some sort of terminal disease and was dying from it anyway) ... Amy survives despite having severe injuries cause Maggie's spirit brought her back to life (after kissing her) ... Maggie continued to be around Amy after the whole ordeal, becoming like some sort of guardian angel lookin after her ... no one can see her of course, except for those who are nearing death, the same case as it is with Charlotte's ghost (explain below)...

----- the main principle is that; actually only people who are going to die soon can see Charlotte... either it is due to some natural causes, such as illness, inccident or whatsoever, or that the ghost herself is going to kill them... there are a few examples in the show... such as Maggie herself, she has some kind of terminal disease which is slowly eating her life away ... then another little boy, who suffers a fracture bone... he dies jus some time after he gets discharge from the hospital (relapse, inccident or some other misfortunate)... and Amy herself ...she is not suppose to die but she gets in Charlotte's ways one too many times in a bid to find out the truth and stop her from harming the children...

--- in case ur interested to know who else survives... the answer is everyone except for two kids, Roy and Simon ... the kids because Charlotte wants them (she likes children, remember) and the two adults because they sort of incur her wrath (directly or indirectly), Roy because he touches something that belongs to Charlotte, and as for Simon, he was trying to help take the children out of the hospital near the end, thus Charlotte kills him when she had a chance alone with him...
