If you are not strictly looking for movies that resemble/are porn, here are a few, several with at least what is currently labeled as "graphic nudity", (translation: male).



SHORTBUS (already mentioned in other comments, plenty of totally unsimulated sex, of many varieties. Art? Cult?)

IN THE REALM OF THE SENSES (older Japanese movie, widely banned when released, recent Criterion release decades later, unsimulated and plenty to offend everyone - "In The Realm Of Passion" is a very cut version of the original)

MA MERE (French movie, ("My Mother"), definitely raw, including some long scenes)

WILD ORCHID (another older movie, set in Rio de Janiero, rumored to have had unsimulated relations between the stars "for realism", also not requested by the director, (maybe just to create "buzz" to sell tickets) and reportedly the first to show "graphic nudity" in a mainstream USA movie, but in a dark scene)

FLESH + BLOOD (older movie, very recently released on Blu-Ray, Medieval setting during plague times, with Rutger Hauer)

THE SAILOR WHO FELL FROM GRACE WITH THE SEA (the scenes with Kris Kristopherson were widely believed to be responsible for his wife divorcing him, possibly tame by contemporary standards)

SIRENS (something of an art movie, with more than a few twists and a few trysts, plenty to be offensive. Take a model/Sports Illustrated Swimsuit model and add possibly 40 pounds gained just for this movie, plus other models for a famous artist (painter), creating 'troubling' paintings...)

SHOWGIRLS (originally devastated by the critics, then labeled as a cult movie, but very popular (eventually), as 'soft porn' and plenty of offensive material/ideas - observation: the actresses are a slightly "fluffy" compared to the actual Las Vegas dancers/extras)

THE STORY OF O (another older movie, reportedly the book was superior, and some of the book's elements didn't get into the movie well, or at all)

JADED(1998) ("Not every sexual predator is a man" is the movie's tag line, which was shocking at the time. Now there are women teachers being jailed for taking advantage of their student(s))

BODY DOUBLE (murder mystery based around porno movies, and one star in particular)

LOLITA (1997, Frank Langela (the original sexy Dracula?) version. Not so much of interest, but there is a scene near the end with a character running down a hallway in an open robe - "shock value")

and then there are some episodes of the various "TV/cable" series... many/most don't do the "graphic nudity" (think of the series GIRLS for one, TRUE BLOOD, CALIFORNICATION, and many others...)
