what was the point of the second premonition

I've only seen 1-3, and each time the main character gets a premonition... although has it ever been revealed who...or what sends these people the vision of their deaths? Or even why.

Anyway, each time they see the accident that will kill them they have the chance to avoid it. Whether it was getting of the plane, the rollercoaster, or the highway. But by the time Wendy saw her death it was too late to avoid. It's almost as if Death sent her the vision of the train crash just to force her to live through it twice as "*beep* you" for having to go to the extra effort to kill the people that got off the rollercoaster.

If Death is sending the premonition in the end, is he also the one sending it in the beginning...why?

I thought that they left the ending ambiguous because they pulled the emergency brake before the accident meaning that they might have survived.

Mr. Death usually has all of his bases covered but I thought it was a bit much that in the first showing of the accident, the train didn't even slow down after going through about 10 support columns and skidding on its side.
