Very hard to answer this- time travel films always leave a lot of questions, to a certain extent we just have to suspend our disbelief, they can never make total 'sense' because of the way we view time as linear (although scientists have proved its not, we as humans find it hard to view any other way!).
To my mind, for time travel to work/ either there are infinitesimal time lines all running at the same time, with tiny little differences. So, in one of these scenarios yes keanu will be dead :(
Or if something changes, it wipes out the previous time line. So it never happened.
Still doesn't make sense I know as it had already happened to Sandra!

Boggles the brain I know, so as I said you have to remember that it's a fantasy and therefore anything is possible. They found each other in the end. :)

The reason she was able to save his life is because the movie consistently followed the fact that he was 2 years exactly behind her. Since he had not gone the plaza yet 2 years to the minute in her timeline, she was able to change his by inserting the letter just in time for him to see it in the mailbox. If she had not done that, everything would of remained the same.

He was always gone in her time because that's when the movie timeline began.
She met with his brother in the morning and was able to get the letter to Alex before the time of day he went to the plaza.

I watched it again tonight after a few years and this answer totally made sense to me. thank you so much...Love this movie but the plot hole at the end was bothering me.

It's simple. Just like the tree Alex planted popped up out of nowhere in Kates time, her writing the letter saved his life and as far as everyone around knew, Alex had never died, just as everyone except her most likely thought the tree was always there.

In movies that include time travel it is usual that only the main characters remember changes and that no one else does, because the main characters are altering the timeline.

So basically the moment Alex received the letter (when you hear the mailbox ping while she is kneeling beside it crying) is when the timeline changes, now the brother has never lost Alex, Alex was never dead. She will remember meeting the younger brother, but the younger brother likely won't remember her now because they only met in the timeline where Alex had died.

Everything changes around the two of them, but not them.
