I was under the impression that it was some kind of sexual delicacy--something in the way of a toy or some other device...since the doctor and Charlotte proceed directly to 'try it out' (Charlotte removing her clothes for this).

I think it was Marquis' penis, removed in treatment no. 13.

It can't be the Marquis' penis, because (in the movie dialog), the doctor says it just arrived from Paris.

In Sade's novels, dildos are often stored in boxes. See for instance "La Philosophie dans le Boudoir". I'm pretty sure it is a large dildo.

Though I did suspect that Treatment 13 was castration, I'm not sure the box contained the Marquis' penis. Didn't the doctor appear to attach it to himself? From Charlotte's screaming I thought maybe it was something he strapped on that had God-knows-what attached to it.

I'm so glad I wasn't the only one who thought about this too. It's just another uncertain thing in the movie, like treatment thirteen.

I thought treatment 13 may have been lobotomy, and the box contained a sex toy (earlier on Charlotte is said to be "kinky").

Just watched the DVD and had to go back and check it out frame-by-frame.
I know it ruins the fun (and I LOVED the "Marcellus Wallace's soul" reference) but it's actually a strap on with a beaded phallus.

I'm thinking that it had to be a strap on, because I was thinking the director had suffered Treatment 13 himself, and needed a strap on to do it with Charolette.
