I thought Kelsey Grammer's character was the replacement, by the way he narrated at the end.

Thanks for posting a question about an actual plot point on this board--it's boring to see a hundred posters saying "This movie was good/bad; I loved/hated it." The identity of the replacement is the reason I came here.

Ivan was speaking to "Elliot" on the phone, but there's no Elliot character, so I'm going with Murph as the new enforcer. Since the movie was fairly formulaic, I think it makes sense that one character got a chance to put her broken world back together and the other survivors just dug deeper into the addiction.
