What is really going on -- spoilers

Basically she doesn't know her husband is having an affair with her sister but 2 and 2 together. She wasn't supposed to be there, the champaigne, estranged marriage, come on they're having an affair. The murders though all took place in her head, everything took place in her head, discussing the experimental drug and the symptoms with the patient gives her someone she desperately she needs, someone to relate with since she cannot discuss it with her husband.

Sad movie really, but weak overall.
