Look of the man who saves ZAC

When Zac is on the desert, he is found(saved) by a man. Zac had already lost or thrown away an instrument for the lung(a little pump). Then this man gives the pump back to Zac, who did not seem to be happy with that. On the next day, Zac uses the pump while is being stared by the man, who at that moment had a mysterious smile.
What do you think about this scene?
What do you think about this man?
What do you think about his smile?
Is there any hidden idea in all this stuff?

I don't think the smile was mysterious. I think that when the man picked up the puffer and then found Zack a few kilometres up ahead he had an inkling that it was his, but Zack pretends that it doesn't belong to him and just turns away. The man may then think "Well, I guess it's not his." But then he wakes up and Zack is using the puffer, his smile is kinda like "I knew it was his! Well, I'm glad that he's being true to himself," or something of the sort.

I may be way off base, but that's how I felt about the scene.

I clicked on this thinking that you were gonna talk about how that guy Zack fvcks looks a hell of a lot like "Jesus."

I thought the man was pleased that Zac was using the device because it was a sign that Zac was recovering. When the man found Zac the latter was clearly dying, if indeed his heart hadn't stopped again for the third time. I think this scene relates to the idea of 'God working in mysterious ways', which Zac recalls when he finds the Patsy Cline record in the market shortly afterwards. The chance for that man to find him lost in the desert and rescue him in time was remarkable. As was his mother waking up and needing to splash her face with water at the same moment.

I like what the other poster wrote too, that the man was satisfied the inhaler had been Zac's as he'd thought.

Is there any hidden idea in all this stuff?

For sure but I'd need to reflect on it for a while.

I thought that the smile meant: "You always try to get rid of your 'predestination', of your lung problems and of your sexuality. You may think that you will get rid of all this after a big challenge, but you won't get it! You must at first accept yourself! All of your features are still with you, as you can notice now with your inhaler"
