I mean, I know they are a young couple and the grandpa shouldn't have forced them at such young age, but they are so cute together and I just had a little doubt, he was all the time in love with her or was he in love with his friend, I am confused from the part in which he's in the classroom and they ask him about his first love, she was the one visiting him, right?

So
there's the whole scene where she finds the letter that she wrote him about her coming to visit and all of the photos he either had/or had been looking at...I am pretty sure it was her who he was talking about in the classroom (she probably came to visit often). She had no idea she meant that much to him but even from the beginning of the movie, you can tell they have memories together.
sangmin (main guy) is talking about her because he got the letter which she wrote to him, the girl did not get the story sangmin told in class about his first love because she thought the letter never got to sangmin in the first place, thus in her head, he would not have known that she went up to visit him.
but at the end of the movie, the girl does find out that the letter did arrive to sangmin, thus realising that the love story sangmin told in class was in fact about her. she is his first love.
