Why the paramedic looks scared when she received the twins in the ambulance? What did she see? They look like regular girls.

Because she realized that she was cast in this crappy film.

Agree...as shocked as the paramedic looked, I was expecting something like the children from "Its Alive", complete with fangs and claws. Didn't make sense to me.
