A brief explanation with spoilers

This my opinion of what the film is about. Hope it helps people to better enjoy the film and to understand what the sex scenes might be about.

Th film is a love story. The battle in the title is Marcos and Ana versus the world. The two are fighting the world to allow them to completely love each other. Marcos has taken care of Ana for 15 years. The inner souls of Marcos and Ana love each other. The differences of their social class, age, and physical appearance keep them from fulfilling that love. Their separation leads them both astray into degrading lives of prostitution and kidnapping. The opening oral sex scene ends with a tear from Ana's eye as she is frustrated that she cannot fully arouse Marcos. The bed sex scene of Ana on top of Marcos shows her trying again to physically love Marcos with full sexual intercourse but even that fails and the end of the scene focuses on the inability of Marcos to ejaculate. The sex scene with his wife shows that Marcos is physically capable of sexual excitement. But with Ana it is too difficult to overcome the psychological and societal barriers that separate their superficial lives. Marcos's drastic solution to this dilemma is for them to leave the material world and join up in heaven through death. He gambles that his pilgrimage of suffering on his knees to the basilica will allow his murderous deed to be forgiven. The final scene shows them happily engaging in sex for the first time in heaven. The inner souls of Marcos and Ana are finnally at peace in a perfect union that was meant to be.
