I guess you didn't understand the movie by the end huh?
The dark world that Rose and Sharon enter is basically a limbo version of the town, conjured and controlled by the Darkness entity of Alessa to punish the fanatics that tried to burn her. They do still age in the alternate world I believe.
The Nurse is also being punished for being too curious. Her physical look is most likely a cruel joke by the Dark Entity because everyone else has aged. She might not even be her real self anymore, there isn't enough information.
I think the monologue implied that the nurse was collateral damage from Alessa's unfocused but dangerous rage energy.
