Ming goes crazy because of his guilty conscience because he was hiding his secret and his role in Yan's death. We see him assume Yan's role in the hospital, when the screen montages from Yan pointing the gun at Ming's head, then Ming taking Yan's place.

Also in the session where Ming and Yan are juxtaposed on the chairs in Dr. Lee's office.

I have just watched watched it last night and was completely satisfied with the ending. The woman at the end is Sam's wife in Infernal Affairs 2 whom Ming had affection for. She turned him down when he advanced on her and in retaliation set her up to be killed by rival gangsters. In my opionion this showed that Ming was always unhinged mentally.

But anyways if you want to understand the ending you have to pay attention to two things in the films. Every time the movies opened or when the movie closes there is a Buddhist proverb referring to the Continous Hell. The other thing is at the before the movie ends one of the officers explains that Ming has permanent brain damage and he will never recover.

Now the reason why Mary (Sam's wife) shows up at the end with the gun is because when Ming's Mary showed up to tell him about the baby he becomes aware of her presence by saying "You're here?" which means that he has a chance of coming back. But Mary shoots him with the revolver and he returns back to a vegetable like state. Now at the end there is a proverb regarding the Avici Hell(I can't remember exactly what it stated but it ends with no escape).

So in summary Ming has entered a symbolic version of Avici or Continuos Hell which has five sufferings.When he damaged his brain. His punishment is to be forever trapped in his mind which is Uninterrupted Time, meaning there is no rest. Mary's Ghost and probably Yan, SP Wong, Sam and SP Yeung are there with him which is Uninterrupted Space. And with Mary shooting him with her gun at the end is considered Uninterrupted Life meaning that whenever he tries to regain his mind , Mary will always be there making sure he never has a chance to escape and she will continue to do so forever.

you know, i've always wondered what they really were tapping out in morse code. i remember that part about Yan saying the password for the files was undercover in morse, and what Ming put in the computer was correct. i'm wondering what Yan tapped throughout the movie, and what Ming was too...

I actually watched the movie with a guy from Hong Kong who works with life boats etc. and he told me the morse code meant "She's just arrived." I think this emphasises his mental illness as he believes his ex-wife is involved as a spy for the Triads.
