She's terrified and facing a clearly crazy guy, why wouldn't anyone in that situation stab away at the guy?

My wife would have - red head, green eyes, and Scotch Irish - the guy would have been sliced and diced. (DO NOT mess with mama! :-) )
That's what gets me in a lot of these movies where the damsel in distress, who has the drop on the bad guy, allows him to get close and disarm her. When I start to get ticked off at these scenes, I just remind myself that it's a movie, and if she did take the guy out, there would be less of a dramatic climax.
It is surely enjoyable when, on rare occasions, the snarling "You don't have the guts" bad guy finds out the hard way that the woman does. I've even clapped once or twice.
You answered your own question. If the female were more aggressive and didn't run away as soon as she even slightly incapacitated the villain, she could dispatch him then and there. End of film!
BTW, while in Scotland a while back, my late partner used the word "Scotch" when referring to the people. He was promtly corrected by a Scotsman that "Scotch" is a drink and "Scottish" is used when referring to the people. I know, I know, that's the way we always refer to those with Scottish and Irish ancestry. Just don't do it in Scotland!
