One major plot hole

When he met Dr. Beth in the future she already knew it was him and she should have told him how he dies even if she wasn't sure who he is. how hard it's to tell him he slipped on the ice while he was getting out of the car and injured his head. and why they didn't even bother to bandage his head after that!! that was very stupid from the director & writers of the movie to leave such a major plot hole so they can give us this sad ending for their movie, Jack should have lived at the end of this movie.

It almost seemed like she didn't want him to change his past, which would be really terrible from her, so she didn't tell him. I don't know if this is a plot-hole or an intention. Whatever the case is, I don't like it.

I agree.

But I don't mind it as it all worked out for him.

Not a plot hole!

She knew. She would eventually give him his wish and put him in the jacket and into the morgue box. Part of her is then accepting that at least some of his delusions are truth. So when she sees him several years later, she's not that surprised and during that meeting, eventually decides not to break the chain, change the path that he's on.

Also, it benefits her to do so, since she knows that he eventually convinces her how to fix Babak.
