Question about meds ***SPOILERS***

Hello. I have a question about the meds. Early on in the film Dawn is seen changing the dose of medication on the computer. After this, Don starts communicating. If Dawn was the patient as revealed at the end, who changed the meds? If the meds weren't changed then why did Dawn/Don begin to communicate?

The med student sneaks into the med room and changes his dose of chlorpromazine (aka Thorazine).

I would like to point out that his original dose is listed as 800 mg qid (4 times daily), which is well over the maximum recommended dose. No wonder he couldn't stay awake! She lowered it to 200 mg qid, which is still a high dose but reasonable. My question is, isn't there any actual medical oversight in this place that would have caught the change?

I know, I know, it's just a movie. This is what happens when you do this for a living... I wonder if DA's get this nitpicking while watching Law & Order.
