If you enjoyed the scene with the cow head, why not look up a little film called The Exorcist 2 : The Heretic. It contains a scene with a large man dressed as a preying mantis. Im not sure whether I was supposed to know it was a man in a suit, but it was terrifying nonetheless!
For me the film is a worthy follow up to its predecessor Rosemarys Baby. Yes its that good and ranks up there with the exorcist. Just imagine how good an exorcist sequel could be.
I actually thought Exorcist 2 was the worst of the set. I saw it when I was younger, and it didn't really scare me at all.

the guy with the cow head IMHO was dream imagery (or was it?) caused by the repeated trauma of that wacko woman being a wacko. The whole milk thing was really getting to minami, and it showed. Of course thats a purely psychological way of looking at it, and i perfer not to get into the whole over analyzing films, but you asked so i give the answer freely.
BTW i was wondering if anybody would have taken the old lady up on her offer in the bathroom? upon further reflection i figure i would have jumped her... i mean she couldnt have been over 45ish

Dream imagery, maybe... But at the same time, Minami wakes up holding the message handed to him by the cow head-guy!
As with many other movies (Lynch etc.), there are many possible explanations, but I think the best way to enjoy the somewhat surreal movie is not to ask too many rational questions...
