Basic plot/what's going on?

I am half way through this movie. My basic knowledge of Hindi has allowed me
to understand some, since Telugu has a lot of the same words (or were they just
speaking in Hindi). I am hopelessly lost, I know the general plot, but there are
no English subtitles and I don't know what's happening. I followed the hospital
scene only because I'd seen it done in another film.
