That made no sense to me: How the heck (drunk or not) does someone mistakenly put SQUID INK ice cubes in their drink??! Could he not see the cubes were black, and that his drink turned black surely almost instantly??

It's a metaphor for making unwise choices in life. Ted's made a lot of unwise choices, not least of which inadequately comforting his wife. Ted seems to have plenty of energy for his daughter, but not for her.
Maybe the kitchen was dark, maybe Ted was distracted from grief, or maybe they just kludged the book in the adaption to film.
