They're actors; it's their job to fake stuff. Also, just because some of the sex is real, it does not mean that all of it is.

He had sex with a body double. According to an interview with both Siffredi and the director, Casar pretty much told Siffredi to keep his penis away from her. Siffredi also had a body double...not for the male/female stuff but for the beginning shot where he was supposed to have received a blowjob from a guy. Apparently the director was surprised that he would object, as he's known to be quite lacking in boundaries, but it was the one thing he wouldn't do on screen.
