Everything that's shown is real.
I see it this way: Fung was lying unconscious in the hospital after he was discovered by Jane's father in the pool. In his dreams he entered the underworld (or the "underworld cops'" headquarters - some castle) and tried to rescue Jane from them. Eventually, thanks to the power of their love, the cops agreed to let them go just for this one final evening. In some miraculous way, Fung regained his health and together with Jane appearead at the dance competition. She took part in it... and in some way they ended up alone in her swimming pool and bid each other farewell while the cops were waiting to take Jane back to the underworld. I don't understand while they were doing it there - perhaps water served as some kind of gateway into the underworld. Anyway, while the girl was taken (or sucked into the water), Fung was so desperate not to part with her, that he dived into the water and as a result almost drowned And so we see him remembering this scene as he lays in the hospital unconscious for the second time (apparently somebody must have rescued him again.)
The last scene where Fung's looking at Jane's photo album takes place when he's recovered after the second almost-drowning. He decides to live his life without Jane, keeping memories of her in his heart.
Anyway, I would have like the movie were it not for the ridiculous "underworld" subplot and cheesy special effects.
