I get the premise and plot of the movie, but, I don't understand it. I just don't understand the whole movie in general. Can someone help me to understand it?

Rinko and her husband, Shigehiko, have a marriage of co-existance. They live together but they don't really have any emotional connection to one another.

One day, Rinko receives a call from a suicidal man named Iguchi. During the course of the conversation, Rinko's compassionate words give Iguchi a new sense of purpose and he decides to live.

Iguchi wants to repay Rinko for her kindness but upon spying on her, he's shocked to find out that Rinko is completely incapable of sharing the emotions she coveyed to Iguchi over the phone with her husband. Her hopes, fears, desires and everthing else is bottled up inside.

Therefore, Iguchi takes a "tough love" approach to force Rinko to come to grips with her feelings for her husband and her fears about life (ie. she has breast cancer). Since she can't appreciate anyone if she can't appreciate her own femininity, Iguchi sets out on a quest to force Rinko to come to terms with her own sensuality, under the logic that if Rinko finds herself to be a beautful and desirable woman then her husband will finally view her as such and will love Rinko unconditionally.

Although Iguchi's feelings look to be genuine, his tactics are disturbing and unconventional to say the least. Rinko obviously rebels since Iguchi's blackmail not only rips her from her comfort zone, it also tears at the societal morals that insulate Rinko and allow her to isolate herself from her husband, herself, and society.

Some of the segments are a bit surreal and the movie is literally heavy with symbolism, but at the end, Rinko and her husband finally break down their respective walls and become a loving couple.
